Terms of service.
By booking or attending any service at Axis Sports Clinic Ltd, you agree to these Terms or Service. All treatments are provided by qualified practitioners in their respective fields. Clients are responsible for providing accurate health information and disclosing any injuries, medical conditions, or concerns prior to treatment.
Physiotherapy Services
Physiotherapy services at Axis Sports Clinic Ltd are provided by independent, self-employed physiotherapists (consultants). Clinical responsibility, including assessment, diagnosis, treatment decisions, and record keeping, rests solely with the treating physiotherapist.
Axis Sports Clinic Ltd provides facilities, admin support, and payment processing only. Payments for physiotherapy services are processed by Axis Sports Clinic Ltd acting as a disclosed agent on behalf of the independent practitioner.
Your contract for physiotherapy services is directly with the treating physiotherapist, not with Axis Sports Clinic Ltd. Any questions or concerns about your treatment should be raised with the treating physiotherapist in the first instance.
Before your first physiotherapy appointment, you will be required to review and sign a disclosure and agreement form acknowledging these terms.
Sports Therapy/Sports Massage Services
Sports massage appointments at Axis Sports Clinic Ltd are provided directly by Axis Sports Clinic practitioners. All treatments, assessments, and clinical decisions are the responsibility of the practitioner providing the service.
Clients must provide accurate health information and disclose any medical conditions, or injuries prior to treatment. This includes any changes to health status in between appointment visits. Axis Sports Clinic Ltd and its practitioners cannot be held responsible for any injury or adverse effect resulting from undisclosed information.
Payments
For all first time clients, payment is required upfront in full. All payments for follow up appointments for ‘Sports Massage’ and ‘Follow-Upt’ are taken at the end of your appointment. We accept cash, card and bank transfer. Please note that upfront payments can only be made via card on the online terminal, or through a Stripe payment link.
Health Insurance/Payment Plans
If you intend on claiming the cost of your treatment through a healthcare plan, it is your responsibility to check cover with your insurance provider before your clinic visit. Your entitlement to claim will depend on your individual policy, the practitioner's qualifications, and the practitioner’s professional affiliations. Details of our practitioner’s qualifications and affiliations can be found on our website. Receipts can only be provided by payments taken on our card terminal. We cannot backdate any documents, such as receipts, for treatments in the past.
Cancellation Policy
If you are unable to attend your appointment, please let us know as soon as possible. The required notice for cancellation/rescheduling of your appointment is 48 hours. Any cancellations within 24 hours of your appointment will ensure a 100% charge. Any cancellations within 48 hours, but before the 24 hours of your appointment will ensure a 50% charge.
We hope you can understand that it is rare that we can fulfill these appointments at short notice, this is why we are obliged to charge a cancellation fee for the practitioner’s time to protect a loss of income.
Refund
In the event of notifying us you wish to cancel a first appointment more than 24 hours but less than 48 hours before the appointment date and time, your upfront payment will be partially refunded 50%. If you cancel more than 48 hours before your appointment, you will be issued a full refund to the card used for billing. Please contact us to issue your refund.
